Many of you may ask what is correction Form?

Correction Form is a short form given during physical clearance when a correction has to be effected before one can be cleared.

They are usually filled and submitted at the University Admission Board (UAB) pending the next Senate meeting before such corrections can be approved. If you know you’ve already completed Online Clearance having any of the following issues then you might not be cleared on time (although sometimes it may go unnoticed and you might be cleared):


Note: If you have done your online clearance and you did not upload your JAMB RESULT, You won’t be cleared on your given date (If any) for some people depending on how fast they were with correcting the mistake after they have been logged out.

1.Inconsistency of Names:  As important as this is, majority of freshers having this issue have already completed their online clearance. For example, we are familiar with the conventional format of writing names as
Surname,
First Name
Middle Name
However the style adopted by the University of Benin is First Name, Middle Name, Last Name (Surname). Any of these styles are accepted.

Thus if my name is *Jibola (First Name) Rukevwe (Middle Name) Ciroma (Surname)
, it is also acceptable if it is written as Ciroma jibola Rukevwe. However, it can be debated in Court that *Ciroma Rukevwe Jibola is not one and the same person. Therefore, any document having Ciroma Rukevwe Jubilation or any alteration to the above is one having Inconsistency of name.

To avoid this common mistake, an Affidavit of Correction of Name is required and should be uploaded in the place for Evidence of Name. Otherwise Correction Form is almost inevitable.

2. Inconsistency In Date of Birth: The date of birth as in your Original JAMB Result Slip, SSCE Certificate, UNIBEN Students’ Profile (KOFA) and Birth Certificate/Age Declaration should be the same. Any alteration can lead to delay in Physical Clearance. This can be handled intelligently when noticed to avoid such delays.

3. Unclear Scans:There should be maximum clarity with the documents uploaded to avoid this common mistake. Again, the scan area should span through the important perimeter to be acceptable. Any such unclear upload can lead to one being logged out so as to upload a clear one. Clarity is essential. Ensure the documents are properly cropped before being uploaded.

4. Uploading Documents on the Wrong Field: We always advise for freshers to do their online clearance in UNIBEN since this is a very common mistake if done else where. For instance, the field for Certificate is only for Direct Entry Certificate such as JUPEB Certificate etc. SSCE Certificate should be uploaded in the place for First Sitting or Second Sitting as the case may be as this field is required and cannot be empty. Again, Result Statement is for Original JAMB Result Slip. All other fields are quite easy.

Wrongly Inputted Subjects/Grades on First/Second Sitting Record and Inconsistent Local Government of Origin: The default grades on the First Sitting Record is as inputed during Post-UTME Registration. Before requesting for clearance, you must check through to ensure that the grades are correctly entered. Again, the Local Government Area of Origin as with JAMB, UNIBEN and as in the Local Government of Origin Certificate/Affidavit of Local Government of Origin should be the same.

6. Ineligibility:To be cleared into any department, you should have met the minimum O/L requirements for admission into that department. If this is not the case, depending on the subject(s) failed, you may be relocated to another department with respect to your SSCE Results and JAMB Subjects Combination or outrightly sent back home.

I wish to assert that few persons having one or two of these issues might be cleared if the clearance officer(s) did not notice them.
